            <description><![CDATA[ <span style="color:#003399">5 Health Card Number</span><br />
o With NRS, the HCN upon discharge, must be the same (number and version code), as upon admission.  In Ontario, this is not always the case.  Sometimes the version code changes during the client’s hospital stay.   CIHI is reviewing this situation.  Until this issue is resolved, the HCN entered on the discharge record must be the same number and version code as originally entered on the admission record.<br />

o The postal code on the follow-up assessment can now be different than the postal code on the admission assessment.<br />
