Chatham-Kent Health Alliance
In 1998, the Chatham-Kent Health Alliance was created, forming in partnership between Public General Hospital, St. Joseph's Hospital, and Sydenham District Hospital. The Alliance Agreement consolidated the three hospital foundations and further established a strategic partnership designed to provide state-of-the-art health care today and into the future. Strong public and private sector partnerships, an integrated delivery system, research-based practice, accountability for efficiency, quality and access determined by "outcome based" criteria, and innovation in utilization strategies have helped the Alliance achieve its goals.

Cornwall Community Hospital
Cornwall Community Hospital is a 170-bed acute care hospital operating on two sites in the city of Cornwall, located in Eastern Ontario along the St. Lawrence River. With a staff of over 1000 employees, Cornwall Community Hospital serves acute, in-patient, out-patient and community based clients. The hospital provides a wide range of medical specialties which include but are not limited to: Anesthesia, Dentistry, Emergency, Endoscopy, Medicine, General Surgery, Gynecology, Internal Medicine, Neurology, Obstetrics, Ophthalmology, Orthopedics, Pediatrics, Psychiatry, Rehabilitation as well as Urology. Cornwall Community Hospital was incorporated in 2004 with the amalgamation of the Cornwall General Hospital and the acute care services of the Hotel Dieu Hospital, both with over 100 years of health care service to Cornwall and surrounding area. In 2006, the Canadian Council of Health Services Accreditation accredited Cornwall Community Hospital for the first time since the amalgamation. Dr. Peter Centre West End is British Columbia’s only HIV/AIDS Day Health Program and 24-hour nursing care Residence. The only program of its kind in Canada, Dr. Peter Centre West End is a leader in HIV/AIDS care, particularly for individuals with multiple medical conditions and other life disadvantages.

HRSRH
The Hôpital Régional de Sudbury Regional Hospital (HRSRH) is a regional resource and referral centre for more than 530,000 residents who live across northeastern Ontario. The HRSRH offers a variety of programs and services that meet many patient care needs, with leading regional programs in the areas of cardiac care, oncology, nephrology, trauma and rehabilitation. The HRSRH is moving forward with consolidating all hospital-based services across three sites, onto one site. Once complete, the new hospital will see patients receive treatment in state-of-the-art surroundings, with new operating rooms, a new Emergency Department, and new medical units. The hospital is also evolving into a full teaching facility in collaboration with the Northern Ontario School of Medicine, affiliated with Laurentian University in Sudbury, and Lakehead University in Thunder Bay.

Recently, we worked with in conjunction with the Canadian Health Information Management Association (CHIMA) to develop the Coding Audit Tool, an application intended to provide facility and regional level analysis of data quality and advance the cause of improved health records management processes.

Providence Care
Providence Care is southeastern Ontario’s leading provider of adult mental health treatment and rehabilitation, complex continuing care, forensic psychiatry, geriatric psychiatry, palliative care, specialized geriatric medicine, long-term care and rehabilitation programs. One of Kingston’s university hospitals, Providence Care is a major centre for teaching and research - many of its staff hold joint appointments with Queen’s University, and conduct clinical research to develop cutting edge treatment for its patients. Providence Care was founded by the Sisters of Providence of St. Vincent de Paul in 1861 and continues their legacy by striving to meet the physical, emotional, social and spiritual needs of each person with respect, dignity and compassion. Today, Providence Care is a not-for-profit organization governed by a volunteer Board of Directors and sponsored by the Catholic Health Corporation of Ontario.

Trillium Health Centre
Trillium Health Centre’s two-site model facilitates the delivery of comprehensive services for both inpatient and ambulatory care. Trillium Health Centre - Mississauga houses all the Health Centre’s inpatient services and has 748 acute, rehabilitation and chronic care beds. This location also has a 24-hour Emergency Care Centre. Trillium Health Centre - West Toronto offers the largest freestanding day surgery facility in North America as well as a 14-hour Urgent Care Centre. Both locations are supported by a full complement of diagnostic, laboratory and pharmaceutical services.

In 1995 the town's two hospitals amalgamated to form the current West Parry Sound Health Centre (WPSHC). Four years later, Hospital Services in Parry Sound celebrated its centennial year.
